Extensor pollicis longus tendonitis is a condition that is classically seen in the setting of nondisplaced distal radius fractures and often progresses to tendon rupture. In this study, the following prodrome of impending EPL rupture was identified: tenderness over the EPL tendon and Lister's tubercle and pain with thumb motion. Injury to the abductor pollicis longus can arise due to hand or wrist sprains, causing pain with certain wrist and thumb movements. Another possible cause for pain in the abductor pollicis longus is De Quervain’s tenosynovitis, a condition usually provoked by repetitive strain or overuse of the wrist and thumb. To put it in a nutshell: Every activity that requires long, repetitive or forceful pincer/pinching motions is prone to tighten up the pollicis muscles and thus to trigger pain, especially in your wrist and thumb. To prevent this from occurring, just massage the muscles after using them, and you are likely to be fine. Pain at the side of the wrist beneath the base of the thumb is a symptom of de Quervain's tenosynovitis.

The word pollicis refers to the thumb and so the flexor pollicis longus is the long muscle which flexes the thumb! Origin – Middle anterior surface of the radius. Insertion – Base of the distal phalanx of the thumb. Actions – Flexion of the thumb. Flexion of the wrist. Innervation – Anterior interosseous nerve.

With too much tension or trigger points in the pollicis muscles, you are prone to feel pain when using your thumb, or even at rest if the muscles are affected severely. 2. Attachment Points of the Adductor & Opponens Pollicis. These two muscles run from your wrist bones and your index finger to your thumb. Within this compartment at the wrist is contained the extensor pollicis brevis (EPB) and abductor pollicis longus (APL) tendons. It is manifested by pain at the base of the thumb that migrates up the wrist and forearm. It gets worse with thumb movement or grip.

de Quervain's tenosynovitis is caused by inflammation and swelling of the tendons of the abductor pollicis longus and extensor pollicis brevis tendons at the level of the radial styloid process. This painful condition occurs most commonly between the ages of 30 and 50. It occurs more frequently in women. Triggering of the abductor pollicis longus in association with deQuervain's tenosynovitis.
